Getting Started
We are so excited you are ready to get started on your medically guided weight loss journey!
Please thoroughly read the information below.
All new medical weight loss patients are required to have a consultation with our medical team to verify candidacy for our program.
Your initial consultation:
$199 paid to Empowered Health
$350 paid directly to the pharmacy - For your first month's supply only

You will have a virtual call with our MD, Dr. Montagne or APRN, Kim Cotnoir to thoroughly review medical history, allergies, current medications, etc. They will verify if you are a good candidate to continue. (Note: this may sometimes be required to be completed from home, prior to your in-office consultation with Jen.)

You will have a one-hour in person consultation with our Lead Nurse Jennifer Baker, to discuss any questions you may have, go over pricing, learn injection techniques, and review your dosing schedule. You will have your weight, photos, and measurements taken and uploaded to your patient chart for clinical purposes only.

If you are approved for the program and you decide to go forth, your prescription will be sent to the pharmacy the same day. Additionally, you will receive your take-home kit with material for safe injecting, healthy eating habits, etc. and a FREE bottle of Nurtafol.

The pharmacy will call you to collect payment and your medication will be delivered directly to your house.

You will be rescheduled roughly 1 week out for your next follow-up. This is mandatory.

You must have a consultation with us to establish care, even if you are already on a GLP-1. However, you may remain on the same dose you're already on, you don't have to start back over.

If you are unable to go forward with treatment, or decide not to, there will be a $55 consultation fee due at the end of your consultation.
* Please note blood work will be required within 3 months of your first injection. If you have had routine bloodwork through your primary care provider in the last year, we ask that you request those results from their office and bring them to your consultation, please.*
Your follow-ups:

After your consultation, you will meet with either an RN, or LNA for a one-week follow-up. You will be asked a series of questions regarding any side affects you may have experienced, how your eating habits have changed, etc. You will have your weight taken again, your progress will be reviewed on your personalized app and you'll have the chance to ask any questions. You will not owe anything for your one-week follow-up.

You will be due back in 3-4 weeks for the same kind of follow-up. At this follow up, ​​​​​​​
your next month's supply will be ordered to be delivered to your house. ​This process will
repeat from there on out. Monthly check-ins carry a co-pay of $50 to cover your monthly nurses visit, access to 24/7 care, your personalized app, sharps disposal, etc.

Monthly check-ins remain mandatory to ensure your progress and safety.
